  • RN Coordinator
  • Social Worker

  • Other health care providers as appropriate

Questions about Palliative Care

When is the right time for palliative care?
Palliative care is best introduced in the early stages of illness and can be provided even at the same time as curative treatments. However, the palliative care team can become involved at any stage of an illness. You and your physician will decide when palliative care is right for you.

Is palliative care the same as hospice care?
No. Hospice care is available for those patients with limited life expectancy, while palliative care is appropriate for any stage of a serious illness. Patients may choose to transition to a hospice program at a later date as appropriate.

What services can the Palliative Care Team provide?

  • Pain management
  • Symptom management
  • Assistance with addressing changing goals of care
  • Helping you and your family make difficult decisions
  • Enhanced emotional and spiritual support
  • Information on placing your healthcare wishes in writing
  • Assistance with difficult but necessary conversations with family and physicians
  • Assistance with discharge planning including referrals to community resources as appropriate

Upon consultation, members of the Palliative Care Team will meet with patients and family members to assess individual unmet palliative care needs. Once these needs are determined, the team will work with you and your family to address them.
